Teen · Class II
The girl who hid her smile with her hand
In every school photograph, she covered her mouth with her hand. Not shyness — strategy. She had learned precisely how to obscure her front teeth while still looking as though she was smiling. Eighteen months of Invisalign Teen changed that. She stopped covering her face.
Before treatment — in their own words
She was fifteen when her parents brought her in. She sat quietly during the consultation, letting her mother do most of the talking, but when asked directly what bothered her most about her teeth she gave a clear answer: she wanted to smile in photos without thinking about it. She had been aware of her teeth since primary school, when a classmate had commented. Since then, the hand had become a reflex. Not every moment — but in photographs, whenever someone pointed a camera, the hand came up.
Her parents had been debating when to seek orthodontic treatment for two years. They were concerned about the impact on school, about whether braces would affect how other students treated her. She told them she wanted to start as soon as possible.
The clinical picture
The malocclusion was a Class II Division 1. Overjet measured 8mm — significantly outside the normal range. There was also a deep overbite, with the upper incisors covering approximately 70% of the lower incisors on closure. The upper arch was mildly crowded, and the lower arch was well-aligned. The skeletal relationship was mildly Class II, within the range amenable to orthodontic camouflage without surgical intervention given her age and remaining growth. Importantly, she was still growing, which allowed Class II elastics to work with her natural development rather than against a completed skeleton.
The plan
Invisalign Teen Comprehensive treatment with Class II corrector elastics. The elastics, worn from the upper canines to the lower molars, applied a forward and downward force to the lower arch while simultaneously retracting the upper anteriors. The combination of tooth movement and elastic force produced the profile improvement alongside the intra-oral correction. She wore her aligners and her elastics with remarkable consistency — her compliance tracking indicators remained almost entirely blue at every check. She had clear personal motivation and it showed in her results.
The outcome
After eighteen months, the overjet had been reduced from 8mm to within the normal range. The deep overbite resolved. The upper and lower midlines were aligned. Her profile changed visibly — the lip competence at rest improved as the upper teeth retracted, and the chin gained relative prominence as the lower jaw relationship normalised. She came in for her debond appointment and smiled immediately when she looked in the mirror. Her mother was in the room. Neither of them said much. The Vivera retainer was fitted that same day.
