The woman who was told it was too late
Her previous dentist had told her that braces were only for children. She was fifty-three. She came to Wassan Dental Centre for a second opinion. Dr. Siju started treatment the following month, and twelve months later her crowding was gone.
Before treatment — in their own words
She had thought about her teeth for decades. The crowding had been there since her twenties, progressing slowly as lower teeth drifted and overlapped over the years. She had raised the subject with her general dentist twice and been told, both times, that orthodontic treatment was for growing children and teenagers. It would not work at her age. The bones were set. The teeth would not move.
She came to Wassan Dental Centre because a colleague had shown her a result. She was expecting to be told the same thing again.
The clinical picture
The clinical examination revealed moderate crowding in both arches, more pronounced in the lower anterior segment where late adult drift had caused significant overlap of the central and lateral incisors. Her periodontal status was assessed and found to be healthy with no bone loss — an important prerequisite for orthodontic tooth movement in an adult patient. There was no skeletal component to address. The problem was entirely dental: insufficient arch length managed over a lifetime without intervention.
Adult bone is indeed denser than adolescent bone, but it moves just as reliably with light, continuous Invisalign forces — and sometimes more predictably.
The plan
A straightforward Invisalign Comprehensive treatment plan was designed to resolve the crowding through arch development and selective interproximal reduction. No extractions were required. The treatment used optimised attachments to provide the precise forces needed to derotate and align the lower incisors while simultaneously correcting mild upper arch irregularities. The patient adapted to the aligners within days and reported that no one at her workplace had noticed she was in treatment.
That last point mattered to her. At fifty-three, discretion had been part of why she had waited so long to act.
The outcome
After twelve months, the crowding was fully resolved. Both arches were aligned. The lower incisors that had been overlapping for thirty years were now in correct position. Her gingival health, already good at the outset, was excellent at debond due to improved access for cleaning throughout treatment.
She was fitted with a Vivera retainer and left with one request: that we update the before and after photographs on the practice wall. She wanted other patients her age to see that it was not too late.
